Your Tasks
- Socio-pedagogical counseling and support of patients (m/f/d) during day-clinic stays
- Support in coping with social, professional, and family challenges
- Mediation and coordination of support services in the social environment (e.g., care services, authorities, rehab, housing, work)
- Collaboration in an interdisciplinary treatment team consisting of doctors (m/f/d), psychologists (m/f/d), nursing staff, and therapists (m/f/d)
- Participation in admission, therapy, and discharge processes
- Documentation and reporting
Your Profile
- Completed studies in Social Work / Social Pedagogy (Diploma, B.A., M.A.)
- Desirable: Professional experience in the psychiatric or social psychiatric field
- Knowledge of social law (SGB II, IX, XII) and network work
- Empathetic, responsible, and structured working style
- Enjoyment of teamwork and interdisciplinary collaboration
