To strengthen our psychiatry team at Spitex Uster, we are looking for you from February 1, 2026, or by arrangement as
Spitex - Certified Psychiatric Nurse HF/FH as Case Manager 60-80%
Your main tasks
- Conducting psychiatric needs assessments with InterRAI CMH
- Management, responsibility, and implementation of the care process
- Psychiatric care and treatment of clients in their familiar environment, including complex and acute situations
- Promoting resources and implementing health-preserving measures
- Interdisciplinary collaboration internally and externally (specialist offices, doctors, clinics, and other services)
What you bring
- Certified Psychiatric Nurse HF/FH with at least 2 years of professional experience in a clinical setting
- InterRAI HC/CMH course or willingness to complete it
- Communication skills and enjoyment in working with clients with psychiatric illnesses
- Digital proficiency for daily electronic administration using Perigon
- Willingness to continue professional development
- High independence, good organizational skills, and resilience
- Own car (with mileage reimbursement by Spitex) with driver's license category B
