Under the umbrella of «Center da sandà Engiadina Bassa» (CSEB) / «Health Center Lower Engadine», important regional providers in the fields of healthcare and wellness are taking a common path. In the national park region, you will find a wonderful environment with diverse recreational and sports opportunities.

Long-term care in the Lower Engadine is based on a holistic care and support model. In addition to two care groups, two nursing homes, and Spitex, a counseling center is also part of the care operation. This is a contact point and hub for questions about aging, care, and support and offers a low-threshold information and counseling service. In addition to counseling, case management for all CSEB operations including the hospital is also part of the counseling center's tasks.

As a successor to the previous long-standing position holder, we are looking for a

Responsible Consultant (50-80%)

Main tasks

Information and counseling for internal CSEB as well as external inquiries on topics related to aging, care, and support – including financial and legal aspects

Mediation of relief and support services for caregiving and/or supporting relatives

Planning, coordination, and organization of follow-up care for patients upon return home or mediation of suitable follow-up solutions

Responsibility for admission management in the inpatient Chüra operations in close cooperation with the Chüra management

Promotion of networking and cooperation with various regional social partners

Coordination and promotion of prevention programs in healthcare

Needs-based further development of the position

Requirements

Completed studies (HF/FH) in health or social work

Further training and/or practical experience in case management

Knowledge of the Swiss healthcare system

High communication skills, knowledge of Romansh is an advantage

Resilient, flexible, and independent personality with organizational skills
