Your tasks with us are

• optimal care and advice in the field of palliative care
• case management responsibility and documentation of the nursing process with interRAI HC 
• support for clients and relatives in difficult life situations 
• collaboration with external agencies such as Palliativ Plus, hospitals, and family doctors 
• advice and guidance of nursing specialists in everyday life

What you bring with you

• completed training as a certified nurse HF/FH 
• desirable: further training in the field of palliative care (CAS) and interRAI HC certificate 
• empathetic, communicative personality with an independent way of working 
• driver's license or willingness to complete assignments by e-bike 
•  IT skills in MS 365 are an advantage
