Your tasks
- Mediation of outpatient aftercare services such as assisted living, self-help groups, nursing services, and supply of aids
- Organization of inpatient follow-up measures such as integration assistance facilities or nursing homes
- Application for benefits from nursing care funds, health insurance companies, and other cost carriers
- Establishment and maintenance of regional care networks
- Support in social reintegration as well as initiation of rehabilitation measures
- Coordination of long-term therapy places in cooperation with clinics, cost carriers, patients (m/f/d)
- Advice on social law issues (health and nursing care insurance, severe disability law, guardianship law, power of attorney)
- Support in securing financial and material livelihood
- Participation in ward rounds and multiprofessional team meetings
- Independent conduct of social therapeutic discussion groups
- Active involvement in structured discharge management
Your profile
- Completed studies in social work / social pedagogy (diploma or bachelor)
- Several years of professional experience in psychosocial work, ideally with people with mental illnesses or addiction disorders
- High motivation for work in the psychiatric environment
- Sound knowledge of relevant social legislation
- Independent, responsible way of working
- Strong communication skills and team spirit
- Willingness to cooperate in a multiprofessional environment
