Certified Nursing Specialist HF / FH Psychiatry, 40% - 50% workload
who values and seeks an autonomous and responsible area of work.
Your main tasks:
- Case management and control of the care process in psychiatric and psychogeriatric case situations in the home environment
- Periodic determination of support needs for clients, creation of the assistance and care plan
- Ensuring the professional provision of care services
- Professional guidance and support of other employees involved in care
