To complement our team in the St. Gallen area, we are looking for a Health Specialist EFZ 80 – 100% to start immediately or by arrangement.

In this responsible role, you actively shape the daily lives of our clients, ensure needs-oriented care, and work closely with relatives.

Your tasks

• Implementation of needs-oriented, demand- and professionally appropriate care of clients at home according to the care plan
• Independent planning and organisation of daily life and living situation together with clients and relatives
• Support or assumption of household management
• Careful and complete maintenance of care documentation
• Interdisciplinary collaboration and coordination with external persons and agencies
• Assumption of additional tasks in planning and coordination if interested and suitably qualified
  

What you bring to the position

• Training as a Health Specialist EFZ
• Solid nursing knowledge and interest in professional and personal development
• High social competence with commitment to constructive, interdisciplinary and interprofessional collaboration
• Independent and responsible working style in a complex environment
• Very good IT user skills, valid driving licence category B
