Your Tasks
- Psychosocial individual assistance, psychosocial counseling, mediation/organization/coordination of necessary support
- Applications for various benefits from different providers (e.g., health insurance, pension insurance, long-term care insurance, social welfare offices)
- Networking with those involved in help planning, cooperation partners (e.g., addiction counseling centers, SPDI)
- Organization of post-stationary care (care level, nursing services, care facilities)
- Information, advice, and support for economic security
- Information, advice, and support in the areas of housing, work, and livelihood
- Counseling and organization of legal guardianships, power of attorney
- Documentation, participation in rounds/teams, competent collaboration in the multiprofessional team
Your Profile
- Completed degree in Social Work (B.A.; M.A.; Dipl.)
- IT skills
- Independent, self-reliant, and reliable way of working
- Resilience and ability to work in a team
- Empathy, organizational skills, and strong communication skills
- Experience in the field of psychiatry is advantageous but not a requirement
