Your tasks:
- Individual and targeted outpatient care as well as case management, including careful creation and continuous processing of care documentation
- Advice and support for clients and their relatives in everyday tasks and challenging crisis situations
- Interdisciplinary collaboration with various professionals and institutions to ensure the best possible care
Your profile:
- Completed HF Psychiatric Nursing training or equivalent training with experience in outpatient care
- High social competence and team-oriented working style
- Strong counseling skills and empathy
- Confident handling of PC, tablet, and smartphone
- Driver's license and a own vehicle
