Your Tasks
- Careful and personal advice and support for our patients and their relatives in problem situations
- Participation in anamnesis and assessment (social anamnesis and social diagnostics)
- Cooperation with internal and external partners such as rehabilitation clinics, outpatient services, and facilities
- Information, advice, application, mediation, and organization of services according to the relevant Social Code Book (SGB)
- Multiprofessional exchange with other hospital departments
- Participation in discharge management
Your Profile
- You have a completed degree in Social Work or comparable qualifications (Dipl., BA, MA)
- You have methodological approaches to activating and accompanying assistance for people
- You enjoy and are open to engaging with a wide variety of people
- You build relationships with fun and sensitivity
