We are looking for a
Dipl. Nursing Specialist HF/FH for Initial Assessment (60%)
Your tasks in the initial assessment
- You conduct the initial assessments at the clients' homes.
- You analyze the current situation with clients and their relatives and derive the necessary care and support measures.
- You create the care process.
- You advise clients, answer questions, and address fears.
- You coordinate, where appropriate, with doctors and other health services.
- You hand over a complete client file to your colleagues in the care team.
Your profile
- You are a certified nursing specialist HF/FH.
- You enjoy working independently, are well organized, team-oriented, and flexible.
- You prioritize your work calmly and proactively.
- Thanks to your solid and broad expertise, you quickly and logically analyze complex client situations.
- Collaboration with interdisciplinary teams is important to you.
- You are empathetic and adapt to the different life circumstances of your clients.
- You possess a driver's license.
