The Dielsdorf Health Center is a modern geriatric competence center with around 500 employees from 35 nations. Our services range from counseling for elderly people and their relatives to general and specialized long-term care, as well as home care and support. With over 200 beds, the inpatient general long-term care offering at the Dielsdorf location is complemented by specialized treatment pathways for rehabilitative acute and transitional care, palliative care, psychiatric geriatrics, and a focus on dementia.

• Accompanying and caring for patients upon admission
• Supporting the execution of admission, discharge, and transfer processes
• Maintaining nursing documentation, creating care plans
• Depending on focus: supporting palliative care, co-designing the gerontopsychiatric or rehabilitation process
• Collaboration in a multiprofessional team consisting of nursing, medical service, therapists, and counseling as well as social services
• Taking over daily responsibility

• You have completed training as a Health Specialist EFZ or an equivalent with SRK recognition
• Good German skills in spoken and written form (at least B1)
• You possess strong social skills and appreciate interdisciplinary collaboration
• High sense of responsibility, independence, and flexibility characterize you
• Handling modern tools is part of your daily routine (PC, tablet, smartphone)
• You are an organizational talent
