HF Nursing Specialist with Case Management & Interest in Vocational Training
Your tasks
- Taking over case management and responsibility for the entire nursing process
- Needs assessments, care planning, evaluation, and coordination of care
- Professional support and assistance for clients and their relatives
- Interdisciplinary collaboration with doctors, therapists, and other specialists
- Participation in the training and support of HF students in everyday professional life
- Promoting learners in their professional and personal development
- Ensuring professional nursing documentation
What you bring
- Completed training as a certified HF nursing specialist
- Experience in case management is an advantage
- Interest or initial experience in vocational training
- Willingness to further develop in the field of education
- High level of independence and sense of responsibility
- Resilience, organizational talent, and solution-oriented thinking
- Teamwork skills, strong communication, empathy, and patience
- Driver's license category B
