Your tasks:

• Advise and support patients in family, professional, and financial matters
• Plan professional reintegration or other follow-up solutions together with patients
• Clarify housing options
• Work closely with the team and external agencies

Your profile:

• You have a diploma from HFS or FH in Social Work
• You have experience in the profession
• You are familiar with the Swiss legal and social insurance system
• Strong communication skills, team player, and resilient personality
