About the job
Your main missions will be the following:
- Ensure the follow-up of requests received on the liaison email address or transmitted by phone;
- Analyze the care needs and implement the first measures in the care network if necessary;
- Ensure coordination with the EVAM placement unit and provide possible recommendations regarding the place of accommodation;
- Transmit the necessary elements to the SSM teams who will receive the beneficiaries for the initial assessment in each Usmi;
- Provide regular feedback on the activity to their direct hierarchy;
- Keep all documents/tables related to the activity up to date.
- Actively participate in the development of a partnership with the care network, CFAs, SPOP, and extra-cantonal partners;
- Coordinate their activity with the EVAM referents for health situations that require it;
- Share with the managers the measures aimed at improving interventions with network partners to promote the maintenance of optimal collaboration.
Your assets:
- Holder of a Bachelor in Nursing or an equivalent recognized title, specific training related to the position is an asset;
- Minimum 5 years of professional experience in a liaison activity position;
- Good knowledge of MS Office tools, knowledge of Soarian and Pulse Medica is an asset;
- Good command of French both written and spoken is required, good knowledge of English and German is an asset;
- Ability to work autonomously and interprofessionally, aptitude to manage complex, conflictual, and emergency situations;
- Analytical mind, sense of priorities and organization as well as a good ability to evolve in a constantly changing context;
