What awaits you
In the role of nursing professional, you take over case management for clients. You are responsible for competent, client-centered, continuous, and coordinated care and support as well as for planning and managing the entire care process. This also includes conducting reassessments with the RAI HC instrument for the responsible clients, to assess and register the care needs with the health insurance companies. In the role of daily responsible person, you act as the contact person for all concerns of the clients and the team members regarding daily business. You also work closely with the deployment planner.
What you bring
You have completed training as a certified nursing professional HF/FH and have a professional work attitude. You are professionally competent, resilient, and flexible and enjoy working independently. Your open and communicative personality significantly contributes to good teamwork as well as interdisciplinary exchange with our partners in healthcare. For the role of daily responsible person, you bring organizational skills and find it easy to prioritize. Working with PC and tablet is part of your daily work. For assignments with our clients, you have your own car available.
