Your Tasks
- Mediation of follow-up outpatient assistance such as outpatient assisted living, self-help groups, nursing service, nursing aids
- Mediation in inpatient offers such as facilities for integration assistance or nursing homes
- Application for benefits from nursing care funds, health insurance companies and other cost carriers
- Networking
- Reintegration into society of the patients treated here (m/f/d) as well as mediation and initiation of rehabilitation measures
- Information and advice on social law issues regarding health insurance, nursing care insurance, severe disability law and similar fields
- Initiation of care procedures at the district court and advice on the topic of power of attorney
- Assistance in securing financial and material livelihood
- Participation in ward rounds and multiprofessional team meetings
- Independent conduct of discussion groups
- Discharge management
Your Profile
- Completed studies in Social Work/Social Pedagogy (Diploma or Bachelor)
- Knowledge of the relevant social laws
- Ability to work in a team
