The Coordination and Advisory Office has been strengthening integrated care in the Northern Psychiatry Region for many years. It connects actors at the interface of social and health services and supports affected persons, relatives, and professionals. With the expansion and further development of the offer, we are creating new professional focal points and expanding our team.
Tasks
- Establishing, maintaining, and further developing regional networks at the interface of social and health services, with a focus on child and adolescent psychiatry
- Professional collaboration in projects as well as further development of defined thematic areas of integrated care
- Planning, organizing, and content design of information and exchange formats such as events, network meetings, and newsletters
- Low-threshold counseling for relatives as well as professional advice for specialists in complex care situations
- Close cooperation with internal specialist departments of ipw as well as external network and cooperation partners
Profile
- BSc in Social Work or equivalent degree in health or social services
- MAS/MSc in Social Work or comparable further training advantageous (or willingness to acquire it)
- Several years of professional experience in health and/or social services
- Good knowledge of care structures at the interface of social and health services
- Experience in networking, coordination, counseling, and/or project work
- Experience in the use of digital work and communication tools
- High flexibility and adaptability in dealing with different stakeholders, subject areas, and changing conditions
- Conceptual thinking as well as enjoyment of interprofessional collaboration
