Health Coordinator Development VIVA.

Job Description

• Implementation of care and administration concepts in the new integrated care regions in collaboration with local teams.
• Guidance of health coordinators and local teams in introducing interdisciplinary working methods, network structures, and the implementation of the VIVA promises.
• Participation in the development of new outpatient care models that meet the challenges of the Swiss healthcare system (digitalization, integration of new professional profiles).
• Promotion of an innovation culture in the regions, especially through the exchange and networking of projects and best practices.
• Implementation of evidence-based practices, including shared decision-making with patients.

Qualifications

• Master's degree in nursing science or public health.
• Bachelor's degree in nursing with additional training (CAS/DAS) in care coordination.
• Extensive experience in the outpatient sector, ideally in a medical practice.
• Experience in project management and change management.
• Very good knowledge of the Swiss healthcare system.
• Pragmatic, communicative, and empathetic personality with moderation and networking skills.
• Ability to lead working groups, mobilize stakeholders, and promote a cooperative culture.
• Native German speaker; French or English at an intermediate level.
