What awaits you:
- Independent, competent and resource-oriented basic and treatment care of our clients of all ages with a wide variety of medical conditions in the communities of Dornach, Gempen and Hochwald
- Taking on case responsibility
- Responsible for planning, managing and monitoring the care process
- Conducting assessments and location discussions with RAI-HC
- Supporting our clients in a self-determined everyday life at home
- Interdisciplinary collaboration with relatives, doctors and other services
What you bring:
- Completed training as a certified nursing specialist HF
- Professional experience and broad expertise
- An independent, responsible way of working
- Openness, flexibility and empathy
- Willingness to be on the road by car
