The Dielsdorf Health Center is a modern geriatric competence center with around 500 employees from 35 nations. Our services range from counseling for elderly people and their relatives to general and specialized long-term care, as well as home care and support. As the largest public Spitex in the Dielsdorf district, we provide outpatient services for our nine contract municipalities. With our comprehensive care offerings, we enable people to stay in their familiar home environment as long as possible despite health limitations. In the future, it is planned to establish a night Spitex.
Independent execution and autonomous planning of the holistic care process for the clients assigned to you in coordination with your colleagues
Scheduling and organizing your work during the day from Monday to Friday
Responsibility for conducting assessments with Inter RAI-CMH, psychosocial care and support of chronically and mentally ill people, as well as conducting conversations and recovery-oriented accompaniment of therapeutic processes
Building long-term nursing relationships with people throughout our entire care area
Intensive exchange, also beyond the boundaries of the Dielsdorf Health Center, with relatives, social workers, the child and adult protection authority (KESB), specialist offices, doctors, etc.
Active participation in the establishment and further development of the psychiatry Spitex
Completed training as a Dipl. Nursing Specialist DN I/DN II/HF/FH
Several years of professional experience in the field of psychiatry
Very good German language skills, spoken and written
Proficient IT skills (MS Office, Perigon)
Driver's license category B
Precise, reliable, and independent working style
High service orientation
Networked thinking, organizational skills, and flexibility
