For our Team E located in Plaffeien, we are looking immediately or by arrangement for a
Dipl. Nursing Specialist HF/FH 70%-90% with the option to take on tasks in daily responsibility.
What awaits you
In the role of nursing specialist, you take over case management for clients. You are responsible for professional, client-centered, continuous, and coordinated
care and support as well as for planning and managing the entire care process. This also includes conducting reassessments with the responsible clients using the
RAI HC instrument to assess and register care needs with the health insurance companies.
After careful introduction to the company and the area of deployment, there is the option to work in the
daily responsibility of the team. In this role, you act as the contact person for all concerns of the clients and the team members regarding daily business. You work closely with the team management and the deployment planner.
What you bring
You have training as a certified nursing specialist HF/FH and a professional
work attitude. You are professionally competent, resilient, and flexible and enjoy working independently. Your open and communicative personality contributes significantly to good teamwork as well as interdisciplinary exchange with our
partners in healthcare. For the role as daily responsible person, you bring organizational skills and find it easy to prioritize. Working with PC and
tablet is part of your daily work. For assignments with our clients, you have your own car available.
