Your area of responsibility
Pure variety: you take responsibility throughout the entire care process – from consultation to care, always with the person in focus.
Teamwork and trust: You work closely with doctors, institutions, and relatives – and of course with your team.
At home instead of hospital: you care for our clients in their familiar environment – especially in complex situations in the field of palliative care. This includes 3-4 night background shifts per month.

Your profile
Education: as a certified nursing specialist HF you bring this (or equivalent training)
Specialist training: in the field of palliative care / possibly support in obtaining B2
Professionalism: combined with a lot of empathy and personal responsibility.
Driving ability: Driver's license category B and own car.
Nice to have: Knowledge of interRAI HC and/or Perigon.
