As a Nursing Specialist HF/FH and Case Manager

you independently coordinate and organize holistic care
you advise and support elderly and young people and their environment - often over a longer period
you clarify and check the needs with interRAI HC and create the care plan with NANDA diagnoses
you plan treatment care in consultation with doctors and therapists
you ensure a high quality of care and service
