About the job
The Dielsdorf Health Center is a modern geriatric competence center with around 500 employees from 35 nations. Our services range from advisory services for elderly people and their relatives to general and specialized long-term care as well as home care and support. With over 200 beds, the inpatient general long-term care offering at the Dielsdorf location is complemented by specialized treatment pathways for rehabilitative acute and transitional care, palliative care, psychiatric geriatrics, and the focus on dementia.
- Accompanying and caring for residents with a focus on palliative care
- Support in carrying out admission, discharge, and transfer processes
- Management of nursing documentation and creation of care planning
- Support in palliative care and co-design of gerontopsychiatric or rehabilitation processes
- Collaboration in the interdisciplinary team (nursing, medical service, therapists, as well as counseling and social services)
- Assuming daily responsibility
- Completed training as a Health Specialist EFZ or an equivalent with SRK recognition
- Good German skills in spoken and written form (at least level B1)
- High social, methodological, and professional competence
- Up-to-date IT skills
- Independent and responsible working style
