The Dielsdorf Health Center is a specialized nursing institution with around 430 employees in the Zurich lowlands and ensures care at nursing places for the 22 political municipalities of the Dielsdorf district with approx. 90,000 inhabitants. With over 200 beds, the inpatient offer of general long-term care is complemented by specialized treatment pathways for rehabilitative acute and transitional care, palliative care, psychiatric geriatrics, and the focus on dementia.
The Regional Spitex forms an important part of the care offer. It provides outpatient services for the five contract municipalities. In the future, it is planned to establish a night Spitex.
- Accompanying and caring for patients upon admission
- Carrying out admission, discharge, and transfer processes
- Conducting geriatric assessment instruments
- Maintaining nursing documentation, creating care plans
- Depending on the focus: ensuring palliative care, co-designing the gerontopsychiatric or rehabilitation process
- Collaboration in the multiprofessional team consisting of nursing, medical service, therapists, and counseling as well as social services
- Taking over daily responsibility
- You have completed training as a nursing specialist HF or an equivalent with SRK recognition
- Good German skills in spoken and written form (at least B1)
- You have pronounced social skills and appreciate interdisciplinary collaboration
- High sense of responsibility, independence, and flexibility characterize you
- Handling modern aids is part of your daily routine (PC, tablet, smartphone)
- You are an organizational talent
