Your area of responsibility includes patient process management with a focus on planning and organizing aftercare. This means:

• Ensuring interdisciplinary information flow, participating in patient rounds
• Organizing and leading conversations with the patient, relatives / contact persons for decision-making and planning of aftercare
• Advising and networking the services of outpatient and inpatient stakeholders such as Spitex / nursing homes / rehabilitation clinics / social services / Pro Senectute
• Organizing nursing home admissions including clarifying finances
• Registering rehabilitations and obtaining cost approvals
• Organizing aids
• Conducting discharge interviews, ensuring information flow and medication process

• Completed training as a certified nurse with professional experience
• Independent working style and organizational talent
• Extroverted and trust-building personality with assertiveness
