Your tasks

• Accompanying and advising clients and their relatives to enable adapted and continuous care at home in stable situations
• Maintaining nursing documentation according to competencies
• Case management assistance
• Interdisciplinary collaboration
• Cooperation with doctors and service providers
• Medication management

Your profile

• Health Professional with EFZ diploma
• Initiative, flexibility, and enjoyment of outpatient nursing activities
• Driver's license category B
• Good PC user skills
