Your Tasks
You work in transitional care and care for and support our clients after acute illness, accident, or chronic diseases, with the goal of accompanying them back home or to a new suitable living environment. You actively take co-responsibility for the design and implementation of the care process and also contribute to a smooth daily routine for the residents and their relatives. Furthermore, you act as an important contact person and ensure the management of electronic care documentation and the application of the RAI system concerning care standards.
Your Profile
- Education at secondary level (FaGe, DNI, FASRK, elderly care worker, home care worker, etc.)
- At least 2 years of professional experience in acute care
- Flexibility and resilience
- Conscientious working style
- Ability to work in a team
- Understanding and empathy for elderly people
