In this role, in addition to basic and treatment care for our clients, you are also responsible for the initial consultations and needs assessments. Through case management and your diverse tasks, you get to know the clients as well as our employees very well and ensure a high quality of care. In the office, you create prescriptions as well as care plans in Perigon and are the contact person for doctors, relatives, and health insurance companies.

• Completed studies as a nurse HF, FH, DNI, DNII or a foreign diploma with SRK recognition
• Initial professional experience, preferably in outpatient or rehabilitation care
• Structured and self-organized working style
• Creativity, resourcefulness, and determination to implement new ideas
• Driver's license, category B

• At least 5 weeks of vacation (age-dependent) and the possibility to purchase additional vacation days
• Possibility for home office
