We are looking for a

Certified Nurse HF/FH 70-100% with Case Responsibility

You bring with you

• Excellent professional and practical knowledge with at least 2 years of professional experience
• An empathetic and enthusiastic personality
• A high sense of responsibility and reliability
• Enjoyment of being on the move by car or e-bike
• Computer skills

Your tasks

• Taking over case management with responsibility for the entire nursing process
• Active participation in basic and treatment care in rapidly changing, complex care situations
• Assessing and clarifying care and support needs using the interRAI HC assessment tool
• Interdisciplinary collaboration with relatives, all involved professional groups, and external institutions
