Role description
- Help and care for the patient, support them throughout their home care
- Participate in assessing needs to establish a precise care plan
- Work independently with the patient and take appropriate initiatives
- Ensure adapted handovers for optimal follow-up and good communication with the team
- Participate in coordinating care with the multidisciplinary team
- Work hours in the morning, during the day, evening, and weekends
- Participate in on-call service
- Be mobile to work in the Chablais Vaudois and Valaisan region: from Villeneuve to Sion
Your profile
- Excellent listening and communication skills
- Ability to assess complex situations
- Sense of responsibility, autonomy, and ability to take initiatives
- Knowledge of home care is an asset
- Ability to collaborate within a multidisciplinary team
Required documents
- Bachelor HES-SO in Nursing or equivalent or foreign diploma recognized by the Swiss Red Cross
- Hold a driving license and own a vehicle
