About the job
The Dielsdorf Health Center is a modern geriatric competence center with around 500 employees from 35 nations. Our range of services extends from counseling for elderly people and their relatives to general and specialized long-term care as well as home care and support. With over 200 beds, the inpatient general long-term care offer at the Dielsdorf location is complemented by specialized treatment pathways for rehabilitative acute and transitional care, palliative care, psychiatric geriatrics, and the focus on dementia.
- Accompanying and caring for patients upon admission
- Carrying out admission, discharge, and transfer processes
- Conducting geriatric assessment instruments
- Maintaining nursing documentation, creating care plans
- Depending on focus: ensuring palliative care, co-designing the gerontopsychiatric or rehabilitation process
- Collaboration in a multiprofessional team consisting of nursing, medical service, therapists, and counseling as well as social services
- Taking over daily responsibility
- You have completed training as an HF nursing specialist or an equivalent with SRK recognition
- Good German skills in spoken and written form (at least B1)
- You possess pronounced social competence and appreciate interdisciplinary collaboration
- High sense of responsibility, independence, and flexibility characterize you
- Handling modern aids is part of your daily routine (PC, tablet, smartphone)
- You are an organizational talent
