We enable the residents of Adliswil, Thalwil, Kilchberg, Rüschlikon, and Langnau am Albis to live a self-determined life in their familiar environment. Nursing Specialist HF/FH 60-80%
Your tasks
- Needs assessments according to interRAI-HC
- Independent, responsible care and support of clients
- Taking over case management
- Assessing care needs and care effort
- Creating, adjusting, and evaluating care documentation according to interRAI-HC
- Performing and documenting interventions according to care planning
- Conducting counseling sessions
- Ongoing review of the quality and effectiveness of care
- Collaboration with relatives, doctors, hospitals, nursing homes, and other specialist agencies
Requirements
- A completed education (HF/FH)
- Professional, independent working style
- Broad professional experience
- Social competence, resilience
- Flexibility, especially regarding working hours
- Good PC skills
- Driving license category B
