Your tasks with us are

• case management responsibility for the entire nursing process control of a client group with the interRAI HC instrument (including professional discussions, case conferences, etc.) 
• ensuring holistic support, care, and nursing of our clients at home
• documentation of nursing-relevant information 
• collaboration and information exchange with external bodies such as hospitals, general practitioners, pharmacies, etc.
• daily responsibility

What you bring with you

• completed training as a certified nursing specialist HF/FH 
• further training and experience in handling the interRAI HC instrument is an advantage
• empathetic, communicative personality with an independent working style 
• driver's license or willingness to complete assignments by e-bike 
• IT skills in MS 365 are an advantage
