The Dielsdorf Health Center is a modern geriatric competence center with around 500 employees from 35 nations. Our services range from counseling for elderly people and their relatives to general and specialized long-term care, as well as home care and support. As the largest public Spitex in the Dielsdorf district, we provide outpatient services for our nine contract municipalities. With our comprehensive care offering, we enable people to stay in their familiar home environment as long as possible despite health limitations. In the future, it is planned to establish a night Spitex.

• Care, support, and counseling of our clients at home

• Ensuring the care process

• Case management and conducting needs assessments

• Creation, adjustment, and evaluation of care documentation

• Conducting counseling interviews

• Accurate recording of service data

• Collaboration with relatives, doctors, hospitals, nursing homes, and other specialist agencies

• Completed training as a Dipl. Nursing Specialist DN I/DN II/HF/FH

• High social, methodological, and professional competence

• Very good German skills in spoken and written form

• Up-to-date IT knowledge (MS Office, Perigon)

• Driver's license category B is an advantage

• Independent and responsible working style
