Your Tasks
- Planning and implementation of holistic and activating social care
- Active collaboration and support of other professional groups in admission and discharge management
- Cooperation with health and nursing insurance companies as well as the responsible authorities, courts, and districts
Your Profile
- Degree B.A. Social Work/Social Pedagogy or other degree in the social law field
- Medical knowledge or basic medical training desirable
- Ability and willingness for reflective communication with patients (m/f/d), employees and supervisors (m/f/d), and other professional groups
- Team and conflict skills and emotional resilience, assertiveness, responsibility, and self-confidence
- Willingness for own further training and education
- Driver's license class B
- Confident handling of PC and office systems
