Your main tasks are
- Independent, competent, and resource-oriented care and counseling of our clients
- Needs assessments (interRAI-HC)
- Management and monitoring of the care process
- Taking over case management in tandem with reference persons

You bring with you
- Completed training as a Dipl. Nursing Specialist HF/FH
- Joy in the profession and dealing with our clients
- Spitex experience and solid knowledge of the interRAI-HC instrument
- Knowledge of the Perigon software is an advantage
