Your Tasks
- Independent care and coordination of chronically ill patients (m/f/d)
- Conducting home visits within the framework of VERAH®/EVA activities
- Organization and implementation of structured treatment programs (DMP) such as diabetes, CHD, COPD, and asthma
- Support of the medical team in everyday practice
- Performing medical tasks such as ECG, blood sampling, vaccinations, and wound care
- Organization of practice processes including appointment management and patient coordination
- Billing according to EBM, GOÄ, BG as well as individual health services (IGeL)
- Careful documentation and administrative tasks
- Patient counseling and training
Your Profile
- Completed training as a Medical Assistant (MFA)
- Additional qualification VERAH® and/or EVA mandatory
- Experience in DMP management and practice organization
- Knowledge of billing according to EBM, GOÄ, BG, and IGeL
- Structured, independent, and responsible way of working
- Friendly and empathetic demeanor towards patients (m/f/d)
- Teamwork, reliability, and organizational talent
- Class B driving license for home visits
