Case Management Specialist / Job Coaching (80%)
Case Management
Individual case management in the support process of participants in the various work integration offers of Streetchurch with the goal of coordinating internal and external measures sensibly and purposefully.
Job Coaching
Support and counseling of participating persons in work integration and beyond during their vocational training and at the "Transition II" from school to work. Maintenance of the employer network and coordination of all involved partners (social security system, disability insurance, vocational schools, employers)
Additionally, this includes
- Cooperative collaboration with internal and external professionals
- Administration, documentation, and reporting
- Active participation in team-, case-, or offer-related meetings
- Education in social work (at university of applied sciences level or equivalent further training) and solid experience in social work
- Know-how and experience in the areas of case management and job coaching
- Knowledge of the Swiss education system, experience with the requirements directed at employees and apprentices in the private sector
- Relationship-oriented and empathetic working style with the target group
- Good administrative skills and written expression
- High independence, initiative, and assertiveness
- Efficient, goal- and result-oriented action
- Social and conflict skills in the team and in dealing with the target group
- Good Microsoft Office skills
