To strengthen our team, we are looking for a

Certified Nursing Specialist HF / FH (40-100%)

Your versatile tasks are:

• You work directly in nursing and take over the advice and guidance of clients, relatives, and employees in complex care situations
• You carry out needs assessments using the InterRAI instrument
  (Costs for InterRAI training are covered by RegioSpitex Limmattal)
• As case management, you are responsible for planning and implementing the nursing process for your clients
• You play an active role in collaboration with the supervising interdisciplinary network and coordinate the corresponding services for your clients

You bring with you:

• You have a degree as a Certified Nursing Specialist HF / FH and ideally relevant professional experience in this field
• You are an open-minded and flexible personality who enjoys engaging with people
• You have good communication skills and are confident in using mobile nursing documentation (smartphone)
• You think in a networked way, work independently and responsibly
