To strengthen our team, we are looking for a

Certified Nursing Specialist HF / FH (40-100%)

Your versatile tasks are:

You work directly in nursing and take over the advice and guidance of clients, relatives, and employees in complex care situations
You carry out needs assessments using the InterRAI instrument
(Costs for InterRAI training are covered by RegioSpitex Limmattal)
As case management, you are responsible for planning and implementing the nursing process for your clients
You play an active role in collaboration with the responsible interdisciplinary network and coordinate the corresponding services for your clients

You bring with you:

You have a degree as a Certified Nursing Specialist HF / FH and ideally corresponding professional experience in this field
You are an open-minded and flexible personality who enjoys approaching people
You bring good communication skills as well as confident handling of mobile nursing documentation (smartphone)
You think in a networked way, work independently and responsibly
