The Dielsdorf Health Center is a specialized nursing institution with around 430 employees in the Zurich lowlands and ensures care at nursing places for the 22 political municipalities of the Dielsdorf district with approx. 90,000 inhabitants. With over 200 beds, the inpatient offer of general long-term care is complemented by specialized treatment pathways for rehabilitative acute and transitional care, palliative care, psychiatric geriatrics, and the focus on dementia.
The Regional Spitex forms an important part of the care offer. It provides outpatient services for the five contract municipalities. In the future, it is planned to establish a night Spitex.
Independent execution of basic and treatment care of residents within the reference person system
Targeted planning and implementation of the care process together with the team
Ensuring and continuous review of the quality and effectiveness of care in the department
Responsible for recording nursing measures (RAI-LTCF assessment)
Monitoring and implementation of medical prescriptions / therapies
Taking over shift responsibility according to the duty roster
Ensuring compliance with and representation of internal instructions such as care guidelines, standards, interdisciplinary decisions, mission statement, and operational norms
Maintaining contact with relatives, doctors, as well as other reference persons of the residents including cooperation with hospitals, homes, and other specialist agencies
Responsibility for sensible, professional, and cost-conscious use and maintenance of materials and devices such as perfusor, infusion pump, pleural drains, PICC and port, as well as tracheostomy and medications
You have completed training as an HF nursing specialist or an equivalent with SRK recognition
You are characterized by pronounced social competence and appreciate interprofessional collaboration
High sense of responsibility, independence, and flexibility characterize you
