About the job
Your main missions will be the following:
- Practice advanced nursing clinical care with the population described in the partnership agreement, in consultation locations and in the community, including at home; perform complete clinical assessments and make diagnoses; prescribe diagnostic tests and treatments according to your scope of practice; ensure follow-up of patients with chronic diseases and develop personalized care plans;
- Develop aspects of prevention, health promotion, and risk factor management for the territory's population;
- Provide clinical support to other healthcare professionals and promote interprofessional collaboration;
- Ensure your own continuing education by following the current cantonal rules and contribute to the training of other healthcare professionals.
- Offer continuing education to other healthcare professionals and promote skills development;
- Collaborate in research related to community health care activities;
- Participate in continuous improvement projects and in optimizing primary care practices, including the sustainability of a community of practice for advanced practice nurses in primary care;
- Support the integration of specialized nurse practitioners in primary care, particularly in the migrant care sector;
- Be a leader alongside the director of nursing in exchanges at the cantonal and federal political levels regarding the role of advanced practice nurses in primary care.
Your assets:
- Holder of a Bachelor’s degree in nursing or an equivalent recognized title as well as a Master’s degree in Specialized Nursing Practice Sciences (MScIPS);
- Certification: Member in good standing of the Order of Nurses of Quebec (OIIQ) and the SNP registry;
- At least three years of professional experience in a similar practice;
- Experience in supervising SNPPL trainees;
- Excellent clinical assessment and diagnostic reasoning skills;
- Autonomy and effective decision-making;
- Good knowledge of the care needs of vulnerable/fragile populations, the care needs of the elderly population aged 75 and over, as well as the care network;
- Very good knowledge of office and computer tools, especially the Office suite;
- Mastery of French and English languages, both oral and written;
- Team spirit and ability to work in interprofessional settings;
- Autonomy and leadership, stress management, and openness to change;
- Adaptability, sense of consensus, organizational skills, and communication abilities.
