These are your tasks

• Taking over holistic, evidence-based care, support, and case management of clients in the outpatient setting.
• Planning and implementation of company-defined care and service measures including quality assurance and documentation (ideally with RAI-HC).
• Interdisciplinary collaboration with doctors, specialist offices, health insurance companies, and authorities – always in the interest of clients and their relatives.
• Additional role as a vocational trainer: accompanying, guiding, and assessing trainees/students according to the educational mandate.
• Active participation in process and service development in a changing organization.

This is what you bring

• Completed training as a certified nursing specialist HF/FH; proven Spitex experience is an advantage.
• Ideally experience with RAI-HC; good German skills in spoken and written form.
• SVEB1 certificate or willingness to obtain it promptly.
• High social competence, empathy, and independent, structured working style.
• Driver's license category B.
