Your tasks with us are
- Case management responsibility for the entire nursing process control of a client group with the interRAI HC instrument (including professional discussions, case reviews, etc.)
- Ensuring holistic support, care, and nursing of our clients at home
- Documentation of nursing-relevant information
- Collaboration and information exchange with external bodies such as hospitals, general practitioners, pharmacies, etc.
- Daily responsibility
What you bring with you
- Completed training as a certified nurse HF/FH
- Further training and experience in handling the interRAI HC instrument is an advantage
- Empathetic, communicative personality with an independent working style
- Driver's license or willingness to complete assignments by e-bike
- IT skills in MS 365 are an advantage
