Case Management
As a Case Manager, you will take over the management of the care process in the area entrusted to you. You will organize and coordinate services in the areas of prevention, health care, or social assistance and align care goals with the direction of the support network and the responsible treating physician. You will advise your clients on dealing with symptoms of illness and provide professional guidance to the nursing staff involved in the care of your team.
Nursing diagnostics and planning
As part of the recurring needs assessment, you will identify current or potential problem areas of your clients using RAI-HC or specific focus assessments. You will then establish the relevant nursing diagnoses based on the POP classification and formulate adapted and realistic care goals together with the affected individuals. In doing so, you will also plan suitable psychiatric nursing interventions that serve to maintain or build the resources of your clients.
Treatment care
With your nursing actions, you will support the implementation of medical therapies in everyday life, such as practicing coping strategies or guiding clients in dealing with aggression, anxiety, or delusions. You will accompany your clients during exposure training or support them in acute crisis situations. Thanks to your presence, you contribute to the prevention of acute self-harm or harm to others.
