For our Team E located in Plaffeien, we are looking for someone to start immediately or by arrangement

In the role of nursing specialist, you take over case management for clients. You are responsible for professional, client-centered, continuous, and coordinated care and support as well as for planning and managing the entire care process. This also includes conducting reassessments with the RAI HC instrument for the responsible clients to assess and register care needs with the health insurance companies.

You bring training as a certified nursing specialist HF/FH and a professional work attitude. You are professionally competent, resilient, and flexible and enjoy working independently. Your open and communicative personality significantly contributes to good teamwork as well as interdisciplinary exchange with our partners in healthcare. Working with PC and tablet is part of your daily routine. You have your own car available for assignments with our clients.
