Home visit and situation analysis with initial clarification of the situation and responsibilities
Advice on illness, support and relief options, social insurances, referral to other counseling institutions
Telephone consultations with relatives/interested parties/informally involved
Round table discussion: organization and leadership, writing minutes
Coordination conference: leadership and presentation of the dossier and joint solution development
Networking with partner organizations and external organizations
Responsibility and execution for the comprehensive implementation of a complete care process within the framework of the key person work
Co-responsibility in preventive, diagnostic, and therapeutic measures
Support in crisis situations
Conducting complex conversations with patients and relatives
Responsibility and co-design of interprofessional collaboration

Degree as a dipl. nursing specialist HF / FH
Experience in caring for mentally ill patients
Very good German language skills, both spoken and written
High level of personal responsibility, independence, and proven social competence
Enjoyment of teamwork and networking with partner organizations
Good networking thinking
