Your tasks
You take responsibility for all steps in the care process of your clients based on RAI-HC needs assessments through case management. In addition to professionally performing care tasks, you delegate tasks appropriately according to the situation and competence to your colleagues. With empathy and patience, you advise relatives and other caregivers and guide them in supportive tasks. You know how to actively collaborate with external specialists, doctors, and other services and represent the concerns of the clients.
Your profile
- Completed training as a certified nursing specialist HF
- Several years of professional experience, preferably in Spitex organizations
- Knowledge of RAI-HC and NANDA nursing diagnostics
- Contemporary IT user skills are an advantage
- Driver's license category B and own car
